To arrange a mental health screening, substance abuse screening or general assessment, submit the form below or call the WellStone Access to Care for your community. Walk-In Assessments are available during regular business hours. If you or someone you know is in crisis, visit our crisis services page.

WellStone serves all patients regardless of inability to pay. We accept Medicare, Medicaid, private insurance, and self-pay. Services are available to clients at discounted rates through a sliding fee scale. Discounts for services are offered based on family size and income.
